Present complain- cough with expectoration since 15 days ..chest pain since 15 days ..some time loss of sensation feeling in upper or lower extremities and trimmers in hands when fever and patient feel weakness since 15 days and fever since 15 days Bcozz of patient sleep outside before 15 days ago ..he feel mild fever every time ..

Tongue- yellow coated

Thrist- normal

Apetetite- loss of apetetite

Stool- normal

Urine- normal

Gait- normal 

Liquor- left 12-13 year ago

Smoke - bidi left 4-5 days ago

Tobacco - everyday

BP- 96/64 mg/hg

Pulse- 78/min

Spo2- 98%

RR- 20/min

Past history- before 13 years ago patient was operated in hospital ..the surgery of intestine infection and patient was operated in hospital and patient get well in 3 months ..surgery Bcozz of excessive intake of alcohol ..now since 15 days ago patient is suffering from chest pain with fever and cough with expectoration and feeling of weakness ..losses motion since 5 days ago ..
