Present complain- cough with expectoration ..more cough when lying ..since 1 year ago ..difficulty in breathing since 1 year ..all problems are aggravated on lying ..itching all over the body since 1 year ago ..backache since 1.5 year ago ..sexual debility and involuntarily secretion of semen with urine every time since 4 years ..which causes weakness ..

Gait- normal
Apetetite- proper
Thrist- 2_3 lit in a day
No smoke
Liquor intake ..occasionally
Stool- normal....1tim in a day
Urine- yellowish/white normal quantity n color
BP- 126/100 mg/hg
Pulse- 72/min
Spo2- 98%

Past history - before 4 years patient have cough with sputum ..breathlessness ..sexual debility with weakness ..body ache ..backache ..itching all over the body since 1 year ago ..patient taken desi treatment but no relief in sexual disorder ..
